Registration Forms

• Supplemental Residency Verification (Verificación de Residencia Suplementaria): This form is used for students who are ‘Doubled Up,’ living with one or more families in the same residence. The homeowner and parent/guardian will need to fill out the form in the office and bring in the homeowners recent utility bill and their Drivers License. 
• Childcare Provider Notarized Letter: This notarized form is used as a supporting document for an Inter/Intra-District request, using ‘Childcare’ as the transfer reason.
